Passed away on Friday, April 25, 2025 at Big South Fork Medical Center

Funeral service will be held at 11 AM on Monday, April 28, at Caanan Independent Baptist Church. Burial will follow in the Mt. Helen Cemetery. The family will receive friends on Sunday, April 27, from 4 PM – 8 PM at Mundy Funeral Home in Jamestown. Family will also receive friends on Monday, April 28 from 9:30 AM until time of the service at 11 AM at Caanan Independent Baptist Church

He is survived by:

Son: Greg Choate and wife Janet

Daughters: Teresa Worrix and husband Jeff, Tammie Hart and husband Bruce

Grandchildren: Alex and wife Katy, Breanna and husband Ethan, Will and wife Taylor, Taylor, Emily, Lane and wife Ashlyn, Lukas, Morgan, Grace, Nevaeh, and Preston

Great-Grandchildren: Lissi, Lexi, Easton, Ellison, and Miracle

He was preceded in death by:

Wife: Lela Garrett Choate

Parents: Porter and Velva Smith Choate
