Passed away on Friday, October 16, 2020 at Signature Healthcare of Fentress County.

Funeral service will be held on Monday, October 19, 2020 at 1 P.M. at the Mundy Funeral Home in Jamestown, with burial to follow in the Storie Cemetery

Family will receive friends on Monday, October 19, 2020 from 11 A.M. – 1 P.M. at Mundy Funeral Home in Jamestown

She is survived by:

Son-in-law: Donnie Choate

Grandchildren: Michael Choate and wife Stacye, Julie Bernhardt and husband Ethan, & David Clark and wife Kelly

Great-Grandchildren: Emma, Eli, Ella, Lily, Landon, Connor, and Nancy
