Funeral Service will be held on Wednesday, July 17 at 12 Noon at Mundy Funeral Home in Jamestown, with burial to follow in the Fentress Memorial Gardens. The family will receive friends Tuesday, July 16 from 5 P.M. – 7 P.M. at Mundy Funeral Home in Jamestown

Sons: Toby Sullivan and wife Trish & Tim Sullivan and wife Melynda

Grandchildren: Jake Sullivan and wife Tyra, Callie Sullivan, Sadie Sullivan

Great-Granddaughter: Josey Sullivan

Sister: Linda Faye Brown and husband Jim
