Passed away on Sunday September 22, 2019 at Cookeville Regional Medical Center

Funeral service will be held on Wednesday September 25, 2019 at 2 P.M. at the Mundy Funeral Home in Jamestown, with burial to follow in the Allardt Cemetery. Military service will be conducted at the grave side by American Legion Post 137

Family will receive friends on Tuesday September 24, 2019 from 5 P.M. - 8 P.M. at the Mundy Funeral Home in Jamestown

Mr. Choate is survived by:

Wife: Reva Reagan Choate

Daughters: Debbie Johnson and husband Jimmy & Linda Asberry and husband Byron

Grandchildren: Lesley Herron and husband Ben, Emily Wheeler and husband Jason, Kelley Porter and husband D.J., Landon Asberry and wife Amber, & Ethan Asberry and Alex

Great-Grandchildren: Drew, Zoe, Mylie, Will, Zak, Noah, & Macklynn

Brother and Sister: Roy & Evelyn

Reva's Children: Linda, James, Joey, Jeffery and their families

Mr. Choate was preceded in death by:

1st Wife: Maurcene Zachary Choate

Parents: Virgil and Hazel Choate

Twin Grandsons: William James and Robert Gene Johnson

Brothers: Cletus, Clayton, R.D., Johnny, Harles, & Herman

Sister: Ellavie
